Case Management Services
Purpose and Outline
The core purpose of this service is to effectively manage:
- Illness, especially musculoskeletal and mental illness, irrespective of an absence.
- Sickness Absence; normally from 1st day of an absence if the absence is likely to be long term, is reoccurring or is a musculoskeletal or stress problem.
- Accidental Injury; if a person is involved in either a work related accident or a social incident then the clinical team will assess whether the injury is compatible with the incident and ensure that the employee is receiving appropriate treatment in order to mitigate employer’s liability claims, lost time or potential future incapacity claims.
- Incapacity; whether the company has a permanent health insurance scheme or a Pension Scheme the purpose is to reduce the cost to the business of Incapacity. The methodology will be dependent on the terms of the contract.
This service will provide:
- Clear and prompt route of referral to the service from a Line Manager or HR Manager.
- Conversation with referring manager regarding reasons for referral and anticipated outcome.
- Telephone assessment and discussion with employee.
- Preparation of initial assessment report and recommendations for next steps including cost benefit analysis if needed.
- If agreement is reached to actively manage the case then the Case Manager will provide the Line Manager/HR with progress updates and any revisions in the case management process via verbal reports and regular written Case Management Reports (CMR’s).
Case Management includes:
- Requests to GP and Specialists for pertinent medical information
- Referral to an Independent Specialist for an Independent Assessment if needed
- Referral for diagnostic services if appropriate
- Referral to an appropriate clinician for treatment
- Occupational Assessment where necessary in more complex cases in relation to fitness to work
- Production of Case Management Reports as and when there is anything to report
- Re-Integration Planning, especially following an episode of long term sick where the return to work needs to be phased
The benefit of providing these services via a central function is to provide access to a broad spectrum of clinical and advisory skills whilst also maximising the efficiency of our administrative processes.
